The Program Coordinator provides assessments, counseling, information, referrals, advocacy services and other resource assistance for Mercy Hospital Carthage opioid elimination program in conjuction with the Pain Clinic.
1. Responsible for the development, organization, and coordination of services including prevention, detection, education and screening programs based on patient needs. This is done in collaboration with Pain Center physicians and other relevant hospital providers.
2. Conduct assessments to evaluate patient's needs and support system to determine appropriate treatment plan. Will establish goals of treatment plan and monitor patient's progress, adjusting as needed to maximize effective outcomes.
3. Facilitates the resolution of barriers to hospital discharge and integration into community and patient/family system. This includes providing appropriate resources, tools and education to assist the patient be successful in their recovery, including their support system in the process.
4. Sources and researches support services available to targeted patients of the program. Maintains sufficient knowledge of these services to assist a patient in making informed choices of care and support upon discharge and during their care outside the hospital.
5. Serves as a clinical/educational resource to hospital staff and within the hospital's service area. Promotes the program through participation in a variety of activities including community events, seminars, civic group meetings, etc.
6. In conjuction with key stakeholders, responsible for the development, implementation, adherence and review of all policies/procedures/processes related to the opioid elimination program.
7. Determines program goals and initiatives along with a plan for monitoring effectiveness. Responsible for adapting these to needs and changes within the program.

Education: MSW degree
Licensure: Licensed as a LCSW in Missouri
Experience: Minimum 2 years experience in position that required diagnostic interviewing skills, counseling skills and techniques, familiarity with local behavioral health resources and development of discharge plans.
Other: Requires excellent interpersonal, organizational and anlytical skills. Must be able to communicate with a variety of people in a variety of settings.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
